TYPES.—Holotype, allotype, and “morphotype,” USNM 81330, 81331, 81332 (I, , II); paratypes, MCZ, USNM, RR.

TYPE LOCALITY.—Ellis Branch, 4 miles west of Carrollton, Carroll County, Kentucky.

RANGE.—Ohio Basin from the lower Kentucky River system to the Salt River, Kentucky.

HABITAT.—Intermittent streams.

TYPE LOCALITY.—Ellis Branch, 4 miles (6.4 km) west of Carrollton, Carroll County, Kentucky.

RANGE.—Ohio Basin from the lower Kentucky River system to the Salt River, Kentucky.
